Transition to Adulthood Worker (Rangatahi)

About the Role

As a Transition Worker, you will provide practical, emotional, and cultural support to rangatahi transitioning from long-term care or youth justice residential placements into adult life. This role is focused on helping young people develop life skills, strengthen their identity, and build meaningful connections that support long-term wellbeing and independence.

You will work alongside rangatahi, whānau, and professionals to create positive outcomes and ensure each young person has the opportunity to achieve their goals in a safe and supportive environment.

Key Responsibilities

  • Support rangatahi to develop life skills, confidence, and independence
  • Assist young people to build and strengthen enduring support networks
  • Encourage and support cultural identity development, particularly for rangatahi Māori
  • Build positive and trusting relationships with rangatahi and whānau
  • Collaborate with external agencies and professionals to support access to services
  • Apply trauma-informed and youth development approaches in everyday practice
  • Support rangatahi experiencing challenges related to mental health, disability, substance use, or behavioural issues
  • Recognise and respond appropriately to concerns regarding abuse and neglect
  • Maintain accurate records and professional standards
  • Contribute positively to workplace culture and uphold health and safety practices

Essential Skills & Experience

  • Relevant qualification in social work, youth work, health, teaching, human services, or similar (Level 6 or above preferred)
  • Significant relevant experience may be considered in place of formal qualifications
  • Experience working with youth or vulnerable people
  • Ability to build rapport and trust with rangatahi and whānau
  • Knowledge and understanding of tikanga Māori, te reo Māori, and Te Tiriti o Waitangi
  • Strong relationship-building and communication skills
  • Understanding of:
  • trauma and adolescent brain development
  • youth development approaches
  • mental health, disability, and substance abuse challenges
  • Behaviour management and conflict resolution skills
  • Knowledge of abuse and neglect indicators and the Oranga Tamariki Act
  • Ability to maintain professional registration where applicable
